I. STATE BUREAU OF IDENTIFICATION (SBI):
- Prints must be taken by the State Bureau of Identification (SBI) for the purpose of obtaining a Criminal History Background check;
- Must have an official form of photo identification;
- Must complete and submit the attached Gaming Control Board's form - Authorization For Release of Information;
- Fee is $61.00 effective July 01, 2006.

II. GAMING CONTROL BOARD:
- Must complete in full and submit the attached 6-page application which includes the Warning Notice Cover Sheet;
- Each page must be initialed by the applicant at the bottom, right corner;
- Application will not be processed without receipt from SBI indicating prints have been taken;
- Last page - Statement of Applicant - must be signed and notarized
- Fee is $15.00 Check or Money Order - Will not accept Cash or Credit Card

FREQUENCY OF MEETINGS: The Gaming Board meets the first Thursday of each month at 1:00 p.m. The meetings are open to the public and are held in the Cannon Building, Conference Room "B", 861 Silver Lake Boulevard, Dover, Delaware. The Board office must receive items submitted for the Board to consider at its meeting no later than two full business days before the meeting. In order to be considered at a Board meeting, license applications must be complete two full business days before the meeting. A complete application is one that includes all required documentation and correct payment.
